Home From Hospital

Independent Living has provided care so older people can continue to live in their own homes since 2011.  Given that most of our customers are elderly it is inevitable that at times they have a stay in hospital.  This may be the result of a planned procedure, a fall, or illness.  However, after a hospital stay many customers want to go back to their own home if this is possible, this may be immediately or in some cases after a respite care in a local care home. Research consistently shows most people want to live in their familiar surroundings.

When a person goes home they may feel initially vulnerable and their health could have deteriorated, however we have the flexibility to put care calls in place.  If someone starts to feel better and regain their confidence we can reduce the number of care calls and/or their length.  If we are visiting a person regularly we can provide feedback to relatives on the state of a loved one’s health.

Home from Hospital Care tasks can include:

  • Keeping people company
  • Helping a person get ready for the day ahead
  • Personal care including washing, dressing showering
  • Prompting medication, food and drink
  • Making meals
  • Light housework
  • Shopping and errands
  • Helping someone to bed
  • Taking a person to appointments
  • Overnight care
